Douglas, hi, I found your site when you commented on my TdF picture in my blog! I am a dentist, but do not have your expertise in cosmetic/aesthetic cases, so I have a question for you. I am contemplating asking my friend to do some work on my teeth. I have two PFMs (8 and 10) that I've had since I was 16 (I am now 44). My chief complaint is that over the years #9 has started to look more and more grey, especially between these two crowns, which as we know are not absorbing any light, but reflecting. My plan is to have the crowns replaced with ???? (Empress?) and get porcelain laminate veneers on 6,7,9,and 11. My goals are to: have no color discrepancys, lighten the overall shade (of course, who doesn't want white teeth), correct very minor overlap at 8-9, minor rotation of 7 with slight <1/2 mm diastema btwn 6-7. However, my number 1 concern is that I get restorations that are proven to be long-lasting (like the amazing crowns I have now, almost 30 years and in mint condition!)

Thanks for offering to share your expertise. Please comment on my situation. I am particularly interested in your opinion of materials you recommend (to use, as well as to avoid), on how strongly you feel about extending to include 1st bicuspids (I don't have extreme display when I smile, but you can see them if I smile wide), and what cement you like (I'm currently using Nexus 2). You can answer me here, on your blog, or at my e-mail which is my blog user name at yahoo.com. (intentionally not typing it due to the amount of e-mail harvesting software crawling around the web.

Hello thank you for your response to my BLOG. Congrats you are the
first person to post on there.Let me get right to answering your questions.

1: Perform bleaching either by trays (preferrably due to more uniform color and control) or Britesmile or Zoom to obtain the base color

2) If bicuspid's color is not acceptable or the bicuspid corridor space is too great then you should include the bicuspids in the treatment plan

3) I find that it is easier to control optical qualities if all the same porcelain is used. If you press the crowns then use the same for the laminates.

4) I prefer feldspatic due to the control of opacity through the use of opacious dentins thereby controlling reflected light and translucency. Procera for the crowns or platinum foil (veneering porcelain that is) for laminates and overlay of coping.

4) USE A GOOD LAB.. Be sure to take photos before and after prep. Full face, retracted and smiling BEFORE ANESTHESIA....Good Luck.Need anymore help let me know.
